Mom Care Docs is a group of family physicians, NOT obstetricians, who provide maternity and delivery care. The patients cared for by our group are considered LOW-RISK maternity patients.

If you have any of the following conditions, you may be considered a HIGH-RISK patient:
-
2 or more cesarean section deliveries (c-section)
-
Diabetes
-
Kidney/Renal disease
-
Autoimmune disease
-
Epilepsy/History of seizures
-
High blood pressure
-
Currently a TWIN/MULTIPLE pregnancy (you must see an obstetrician)

If you answered "YES" to any of these conditions, it is best to please see a family doctor to discuss the most appropriate care for your pregnancy. A referral to an obstetrician may be best for you and your baby. If you do not have a family doctor, please visit the Alberta Health Services website Your Health Care Options for information.

The form is stored within our clinic server and only used for the purpose of booking your appointment.
